It is my understanding that any commercial use of a customer's image requires a model release form for safety.
That being said, I have used a few imaged items for display without one. They are something like an imaged dog tag that had a message engraved wrong on the back, darned employees, I never :>P did that myself. The customers are usually tickled by their image on display. If they were to object it is simple to take it down and that would be that.
I would be more cautious on a website. People think you make a fortune there and would want a piece of the imaginary pie. There I use only pictures of my family and those that came in the item like in frames.
